Change of Patient Details

It is essential for you to notify us of any change of address, name and telephone number.  It is important that we can get in touch with you, should the need arise. Please fill out a form at reception if any of your details change. 

You should also send back any forms requesting conformation of your registry at the practice.  These forms are sent out by the primary care trust and they (not us!) remove patients from our list who do not respond.
